Adult Day Services
Social recreational activities for cognitively impaired and/or frail clients provided on-site at 80 Ward St. This service provides caregivers with a full day of relief.
Meals-On-Wheels
Hot nutritious meals, frozen meals and fruit and vegetable baskets delivered to client homes. Each meal delivery includes a security check to monitors client's well being.
Home Support Program
Home Help program provides light housecleaning, laundry, meal preparation, and assistance with personal care including bathing and transferring.
Respite Care Program
The Respite Care Program offers support, stimulation, and dependable in-home care to frail and/or disabled seniors who cannot be safely left alone; this program provides caregiver relief.
Transportation Program
Provides rides to medical appointments & social appointments for seniors and adults with disabilities who find it difficult to use public transportation.
Case Management
The Case Management Program offers information and referrals to support individuals and their families. Case Managers link individuals to programs and services to help meet their needs, to continue to live independently in their homes. Case Managers also provide assistance in attaining financial assistance, advocacy and connections to other community resources. The Case Management team follows-up with clients to assist them to meet their needs. Long-term Case Management/Counseling services are referred to appropriate external resources.
Caregiver Counseling
This program offers assistance to caregivers to continue to provide and support their loved one. Assistance is provided through support groups, education and knowledge sharing, and linkage to other appropriate resources. This support is offered both on a 1:1 basis as well as through groups.
Community Dining Program
Offers a hot, nutritious lunch away from home at various community venues, and this outing provides an opportunity for socialization and recreation. We also offer a monthly Lunch Date Program at different restaurants and community sites. A great chance to meet new friends!
Friendly Visiting & Telephone Security Checks
Volunteers visit seniors who are isolated and have little or no opportunity to participate in activities outside their home. Regular telephone calls are provided to those clients who require contact.
Grocery Shopping
Clients are driven to and from their home in the agency vehicle to a local grocery store. It's a great opportunity to get out - and get stocked up with food! This service is offered every two weeks.
Income Tax Clinic
Helps low-income seniors file Income Tax forms.
